A comprehensive in-depth needs assessment is provided by professional care coordinators. This assessment includes evaluation of physical, emotional, cognitive, social, financial, legal and safety concerns (including fall risk and home safety assessments). With the client's participation, the care coordinator develops a care plan individually designed to meet the client’s needs. Referrals are facilitated to various community resources, advocacy is offered, and education is provided to clients on topics of concern. At the end of the initial session, the client is given an individually designed informational packet including a written “Summary Report of Findings and Recommended Plan of Care”. The care coordinator then monitors and reassesses the client as needed.
